Phoenix TEGA Door Opening Anomaly

A mechanical interference, attributed to inadequate processes and procedures for instrument design, verification, and validation, prevented the full opening of a set of instrument doors following the landing of Mars Phoenix. The problem was attributed to the instrument contractor’s inadequate documentation of the anomaly, and failure to adequately communicate a redlined design change to a subcontractor. The anomaly also represents a violation of the "test-as-you-fly" principle.
